Rhode Island 5 Day Notice to Pay or Quit | Form DC-55
The Rhode Island five (5) day notice to pay or quit, often referred to as ‘Form DC-55′ § 34-18-35 and § 34-18-56, is the notice that must be served to a tenant who has failed to pay rent in accordance with their lease/rental agreement. The notice can be served once a tenant is more than 15 days late on rent. Once the tenant has received the notice, they have 5 days to pay the past due amount of rent or vacate the rental unit. If the tenant does not comply with the notice, the landlord can file an eviction lawsuit in district court.

How to Write
Step 1 – In the first input field (“Date of Mailing”) enter the date (dd/mm/yyyy).
Step 2 – Enter the name of the tenant in the “Name” box, and then fill in the address of the rental property (street, unit number, city, state, ZIP).
Step 3 – Write the total amount of rent that is due.
Step 4 – Enter your signature, print your name, and then provide a contact/return address.
Step 5 – Fill in the date of service (the date that the notice was mailed to the tenant), and then sign your name in the “Signature” field.
