The Massachusetts thirty (30) day lease termination letter, described in Chapter 186, Section 12, is reserved to be used to terminate a monthly lease agreement. This notice does not apply to when a month-to-month tenant has failed to pay rent (a fourteen day notice should be issued in that case). The lease termination letter informs the tenant (or landlord) of the upcoming termination of the rental agreement. The minimum amount of notice required is thirty (30) days.
How to Write
Step 1 – Put the date of notice, and then full address of the rental unit at the top of the form. In the “To:” input field, enter the name(s) of the tenant(s).
Step 2 – Enter the start date of the lease.
Note: If you are the tenant using this notice to inform your landlord of your intention to terminate the monthly rental agreement, enter an address for your security deposit to be sent to.
Step 3 – In the “Proof of Service” section select the type of delivery method that was used, and then enter the date of service. Sign and print your name at the bottom, and then enter the date.
Note: It’s recommended to make several copies of the completed notice.
