Colorado 3 Day Notice to Quit Form JDF-97 | NonPayment & NonCompliance
The Colorado three (3) day notice to quit, pursuant to § 13-40-104, is used by the landlord to inform the tenant that they must vacate the rental unit/property within three days of receiving the notice. A tenant is typically served this type of notice after repeated lease agreement violations, or when they have committed a substantial breach in the agreement.
How to Write
Step 1 – Download the notice to quit form in .PDF format here.
Step 2 – At the top of the form, to the right of the “TO:” field, enter the tenant’s full name as it appears on the rental/lease agreement.
Step 3 – Enter the approximate date and time that the lease will be terminated at.
Step 4 – Enter the street address, city, county, subdivision, lot, and block where the rental unit is located.
Step 5 – Provide a description for why you are terminating the lease (include specific violations, past offenses, etc.).
Step 6 – Enter the current date, and print/sign your name in the “landlord/owner” field.
Step 7 – Under the “Certificate of Service” section enter the date that the notice was served to the tenant, as well as the county where it was served. Select which type of delivery method was used (personally delivered to tenant/family member over the age of 15 OR posted in a conspicuous location on the premises of the rental unit). Sign your name above the “Signature” field.
